Алкогольную зависимость относят к одной из наиболее часто встречаемых коморбидных патологий, наряду с тревожными расстройствами и расстройствами личности.</p></sec><sec><title>   Цель</title><p>   Цель: определение частоты коморбидности БАР с алкогольной зависимостью у пациентов специализированного психиатрического стационара и выявление клинико-динамических особенностей БАР при данной коморбидности.</p></sec><sec><title>   Пациенты и методы</title><p>   Пациенты и методы: в психиатрическом отделении обследованы 188 пациентов с верифицированным диагнозом БАР по МКБ-10 (136 женщин и 52 мужчины). Возраст пациентов женского пола составил 41,6 ± 14,4 года, мужского пола — 39,4 ± 14,0 года. В зависимости от текущего аффективного расстройства пациенты распределились следующим образом: депрессивный эпизод — 62,2 % (n = 117), гипоманиакальный эпизод — 2,7 % (n = 5), маниакальный эпизод — 1,6 % (n = 3), смешанный эпизод — 33,5 % (n = 63). Проведена сравнительная оценка клинико-динамических характеристик БАР в двух группах пациентов: с биполярным аффективным расстройством и коморбидной алкогольной зависимостью — 30 человек (14 женщин и 16 мужчин), возраст 43 года [29; 55] и без коморбидности с алкогольной зависимостью — 59 человек (39 женщин и 20 мужчин), возраст 36 лет [29; 50]. В ходе исследования использовались клинико-психопатологический, клинико-катамнестический, психометрический, статистический методы.</p><p>   Результаты и их обсуждение: определен уровень коморбидности БАР с алкогольной зависимостью — 15,9 %. Было выявлено, что при БАР с коморбидной алкогольной зависимостью, по сравнению с БАР без алкогольной зависимости, пациенты имеют ранний возраст начала аффективного заболевания, чаще имеют в его структуре смешанные эпизоды. Кроме того, у таких пациентов отмечается более высокий риск суицидального поведения, БАР I типа и обострения аффективной симптоматики. Показано, что алкогольная зависимость затрудняет распознавание БАР при первичном обращении за специализированной психиатрической помощью.</p></sec><sec><title>   Заключение</title><p>   Заключение: присоединение алкогольной зависимости к БАР негативно сказывается на его основных клинико-динамических показателях и отдаляет сроки его диагностики.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>   Background</title><p>   Background: numerous studies indicate a high level of comorbidity of bipolar disorder with other psychiatric disorders. Alcohol dependence is one of the most common comorbid pathologies, along with anxiety disorders and personality disorders.</p></sec><sec><title>   Objective</title><p>   Objective: determination of the frequency of comorbidity of BAR with alcohol dependence in patients of a specialized psychiatric hospital and identiашcation of clinical and dynamic features of BAR with this comorbidity.</p></sec><sec><title>   Patients and methods</title><p>   Patients and methods: in the psychiatric unit, 188 patients with bipolar disorder diagnosed according to ICD-10 (136 women and 52 men) were examined. The age of female patients was 41.6 ± 14.4 years, male — 39.4 ± 14.0 years. Depending on the current affective disorder, patients were distributed as follows: a depressive episode — 62.2 % (n = 117), a hypomanic episode — 2.7 % (n = 5), a manic episode (ME) — 1.6 % (n = 3), mixed episode — 33.5 % (n = 63). A comparative assessment of the clinical and dynamic characteristics of bipolar disorder was carried out in 2 groups of patients: with bipolar disorder and comorbid alcohol dependence — 30 individuals (14 women and 16 men), aged 43 years [29; 55] and without comorbidity with alcohol dependence — 59 individuals (39 women and 20 men), age 36 years [29; 50]. During the study, clinical-psychopathological, clinical-catamnestic, psychometric, statistical methods were used.</p><p>   Results and discussion: the level of comorbidity of bipolar disorder with alcohol dependence was determined — 15.9 %. It was found that in BD with comorbid alcohol dependence, compared with BD without alcohol dependence, patients have an early onset age of BD, more often have mixed episodes during an affective disorder. In addition, such patients have a higher risk of suicidal behavior, bipolar-I, and exacerbation of affective symptoms.
