Предполагается, что этиопатогенез ТРШ отличается от патогенеза шизофрении, восприимчивой к терапии, что, возможно, лежит в основе отсутствия удовлетворительного ответа на большинство антипсихотиков.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: определение морфометрических особенностей серого вещества головного мозга при ТРШ, а также анализ ассоциации этих показателей с клиническими характеристиками пациентов. Пациенты и методы: 21 праворукий пациент мужского пола с диагнозом параноидной шизофрении, соответствующей критериям терапевтической резистентности, а также 21 психически здоровый испытуемый того же пола и возраста прошли клиническое и МРТ-обследование. Т1-взвешенные изображения были обработаны в пакете FreeSurfer 7.1.1, в результате чего для каждого испытуемого были получены средние показатели толщины и площади серого вещества коры, объема подкорковых образований, структур ствола мозга, отдельных ядер миндалевидного тела и отдельных субрегионов гиппокампа. Были проведены межгрупповые сравнения и рассчитаны корреляции с психометрическими показателями (PANSS, CDSS) и дозировкой антипсихотиков в хлорпромазиновом эквиваленте.</p></sec><sec><title>Результаты</title><p>Результаты: у пациентов с ТРШ по сравнению со здоровыми испытуемыми обнаружена меньшая толщина коры в лобной, височной, теменной, затылочной, поясной и островковой областях, а также меньшие объемы миндалевидного тела, гиппокампа и прилежащего ядра, отдельных ядер миндалевидного тела и субрегионов гиппокампа обоих полушарий. Напротив, объем правого бледного шара оказался больше у больных шизофренией по сравнению с нормой.</p></sec><sec><title>Выводы</title><p>Выводы: распространенное уменьшение серого вещества при ТРШ находит подтверждение в литературе, описывающей резистентность как свойство более тяжелой формы шизофрении, сильнее затрагивающей мозговые структуры. Неожиданным результатом оказалось увеличение объема бледного шара, которое пока не находит четкого объяснения.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: despite a significant progress of psychopharmacology, treatment-resistant schizophrenia (TRS) remains a challenge for clinicians. The etiology and pathogenesis of TRS probably differ from schizophrenia susceptible to therapy, which underlies the non-respondence to most antipsychotics.</p></sec><sec><title>Objective</title><p>Objective: to establish morphometric gray matter brain structural features in TRS as well as to analyze the association of these parameters with the clinical characteristics of patients.</p></sec><sec><title>Patients and methods</title><p>Patients and methods: 21 right-handed male patients diagnosed with paranoid schizophrenia and meeting criteria for treatment resistance and 21 matched healthy controls underwent MRI and clinical examination. T1-weighted images were processed via FreeSurfer 7.1.1. For each subject average values for the cortex thickness and area, volumes of subcortical structures, brain stem structures, and separately volumes of the amygdala nuclei and hippocampal subregions were obtained. Intergroup comparisons and correlations with clinical scales (PANSS, CDSS) and antipsychotic dosage in chlorpromazine equivalent were calculated. Results: TRS patients showed decreased gray matter thickness in frontal, temporal, parietal, occipital, cingulate and insular regions, volumes of the amygdala, hippocampus and nucleus accumbens, as well as a number of amygdala nuclei and hippocampal subregions bilaterally. The volume of the right globus pallidus, on the contrary, was increased.</p></sec><sec><title>Conclusion</title><p>Conclusion: the widespread gray matter thinning in TRS confirmed the other researchs, which described resistance as a more severe form of schizophrenia that affects brain structures worse.
