Цель работы — изучение структуры назначений антипсихотиков больным шизофренией в психиатрическом стационаре. Материалы и методы: исследование выполнено в СПб ГКУЗ «Городская психиатрическая больница № 3 им. И.И. Скворцова-Степанова». Тенденции потребления антипсихотиков за период с 2017 по 2022 г. были изучены с помощью теста Манна–Кендалла. Для унификации сравнения потребления препаратов количество действующего вещества в упаковке делилось на его среднесуточную дозу. Сравнение потребления антипсихотиков лечебными отделениями проводилось методом бикластерного анализа. Данные о структуре назначений антипсихотиков получены из 315 медицинских карт пациентов с параноидной шизофренией (56,8% — лица мужского пола, средний возраст 32,9 (8,3) лет). Результаты: потребление антипсихотиков в больнице за период 2017– 2022 гг. характеризовалось снижением доли типичных антипсихотиков (ТАП) до 12,8% за счет увеличения доли атипичных антипсихотиков (ААП) до 61,0% и антипсихотиков длительного действия (АПДД) до 26,2% (p = 0,002). Назначение антипсихотиков в разных лечебных отделениях больницы было относительно равномерным. Клозапин (26,9%), зуклопентиксол (20,0%), галоперидол (10,3%), оланзапин (10,3%), рисперидон (9,3%), кветиапин (8,2%), палиперидон (4,1%) составили 89,1% всех израсходованных антипсихотиков. Суммарная доля карипразина, перициазина, арипипразола, зипразидона, левомепромазина, хлорпротиксена, хлорпромазина, тиаприда и трифлуоперазина, сертиндола, луразидона, сульпирида, флупентиксола и брекспипразола составила 10,9%. Среди назначенных больным шизофренией препаратов лидировали рисперидон (36,2%), галоперидол (17,1%), оланзапин (15,6%), клозапин (10,8%). Частота назначения остальных препаратов была менее 10,0%. Доля ТАП составила 26,3%, ААП — 73,7%. В подавляющем большинстве случаев (98,1%) пациенты получали монотерапию. Вывод: полученные данные о структуре назначений антипсихотиков демонстрируют соответствие представленных подходов мировому тренду преимущественного применения антипсихотиков второго поколения при стационарном лечении шизофрении.</p></abstract><trans-abstract xml:lang="en"><p>Results: consumption of antipsychotics in hospital for the period 2015–2022 characterized by a decrease in the proportion of typical antipsychotics (TA) to 12.8% due to an increase in the proportion of atypical antipsychotics (AA) to 61.0% and long-acting antipsychotics (LA) to 26.2%. The administration of antipsychotics by hospital treatment units was relatively uniform. Clozapine (26.9%), zuclopenthixol (20.0%), haloperidol (10.3%), olanzapine (10.3%), risperidone (9.3%), quetiapine (8.2%), paliperidone (4.1%) accounted for 89.1% of all antipsychotics consumed. The total proportion of cariprazine, pericyazine, aripiprazole, ziprasidone, levomepromazine, chlorprothixene, chlorpromazine, tiapride and trifluoperazine, sertindole, lurasidone, sulpiride, flupenthixol and brexpiprazole was 10.9%. Among the medications prescribed to patients with schizophrenia, the leading ones were risperidone (36.2%), haloperidol (17.1%), olanzapine (15.6%), and clozapine (10.8%). The frequency of prescription of other drugs was less than 10.0%. The share of TA was 26.3%, AA — 73.7%. In the vast majority of cases (98.1%), patients received monotherapy.
