Наибольший интерес представляет псориаз как мультифакторное кожное заболевание, обладающее общими кластерами генетического риска с таковыми при ряде психических расстройств, в частности, аффективного спектра. Манифестация одного из них может оказать влияние на реализацию другого, повышая и без того высокий риск снижения трудоспособности, инвалидизации и значимого ухудшения качества жизни, характерных для заболеваний в отдельности. Исследования иммунологических аспектов взаимосвязи псориатического процесса и психической патологии позволят оценить возможность использования иммунобиологической терапии в качестве патогенетической терапии как соматических, так и психических нарушений. Цель обзорной статьи — на основании научных публикаций о результатах исследований коморбидных псориазу психических расстройств разработать методологию и дизайн изучения этих нарушений в динамике с учетом применения препаратов иммунобиологической терапии. Материалы и методы: поиск проводился в базе данных PubMed, Mendeley и ScienceDirect с использованием поискового запроса по ключевым словам: “comorbidity”, “mental disorders”, “psoriasis”, “immunobiological therapy”, “immunophysiology”. Заключение: многочисленные исследования подтверждают наличие общего патогенетического звена в развитии псориаза и ряда психических расстройств, однако на настоящий момент отмечается недостаточное количество исследований, посвященных изучению общих иммунологических показателей в структуре данных состояний на больших выборках. Помимо этого, в работах, направленных на оценку динамики психической патологии в условиях применения иммунобиологических препаратов, фактически отсутствует клиническая оценка психического статуса пациентов с верификацией данных, полученных с использованием психометрических методик, что также затрудняет адекватную оценку перспектив использования биологической терапии в практике. </p></abstract><trans-abstract xml:lang="en"><p>Background: mental pathology is widely represented among patients with skin diseases. Psoriasis is of greatest interest as a multifactorial skin disease that has common clusters of genetic risk with those of a number of mental disorders, in particular, the affective spectrum, while the manifestation of one of them can have an impact influence on the implementation of another, increasing the already high risk of decreased ability to work, disability and significant deterioration in the quality of life, characteristic of individual diseases. Studies of the immunological aspects of the relationship between the psoriatic process and mental pathology will allow us to evaluate the possibility of using immunobiological therapy as a pathogenetic therapy for disorders in both the somatic and mental spheres. The aim of this review is to substantiate the methodology and design of a research aimed to study mental disorders comorbid with psoriasis over time, taking into account the use of immunobiological therapy drugs. Materials and Methods: the search was conducted in the PubMed, Mendeley and ScienceDirect databases using a search query on keywords and terms (and their derivatives) for: “comorbidity”, “mental disorders”, “psoriasis”, “immunobiological therapy”, “immunophysiology”. Conclusion: numerous studies confirm the presence of a common pathogenetic link in the development of psoriasis and a number of mental disorders, but at the moment there is an insufficient number of large studies devoted to the study of general immunological parameters in the structure of these conditions.
