Актуально изучение влияния коморбидности на клиническую картину и тяжесть проявлений ПТСР.</p></sec><sec><title>Цель исследования</title><p>Цель исследования: анализ структуры и тяжести клинико-психопатологических характеристик ПТСР с учетом коморбидности и установление взаимосвязей с интенсивностью субъективного переживания травматического события.</p></sec><sec><title>Пациенты и методы</title><p>Пациенты и методы: обследованы мужчины, участники СВО (n = 52) в возрасте от 22 до 55 лет (средний возраст 38,2 ± 8,9 года), самостоятельно обратившиеся и прошедшие стационарное лечение в связи с ПТСР, коморбидным с другими психическими расстройствами (по критериям МКБ-10). Диагноз ПТСР дополнительно верифицировался международным нейропсихиатрическим интервью (Mini-international neuropsychiatric interview, M.I.N.I.) для ПТСР. Тяжесть клинических проявлений ПТСР и интенсивность влияния травматического события оценивали с помощью Миссисипской шкалы ПТСР (Mississippi Scale for PTSD) и Шкалы оценки влияния травматического события (Traumatic Event Impact Scale-R, IES-R; ШОВТС).</p></sec><sec><title>Результаты</title><p>Результаты: в структуре коморбидных с ПТСР психических расстройств выявлено преобладание других тревожных расстройств, расстройств адаптации (n = 24; 46,2%) и органических психических расстройств (n = 12; 23,1%). Проведенный факторный анализ симптомов ПТСР позволил выделить четыре группы факторов, отражающих: (1) интрузивные проявления с выраженной тревогой, повторностью переживания и избеганием травматического события; (2) негативное эмоциональное самовосприятие; (3) неспособность испытывать положительные эмоции; (4) нарушения сна с гипервозбуждением. Установлена статистически значимая наибольшая интенсивность влияния травматического события на личностный профиль участников боевых действий и тяжесть проявлений ПТСР в случаях коморбидности с тревожными, аффективными и личностными расстройствами (p = 0,00001). Коморбидность с органическими психическими расстройствами характеризуется меньшей тяжестью клинических проявлений ПТСР и меньшей интенсивностью влияния травматического события на личностный профиль (p = 0,00001)</p></sec><sec><title> Заключение</title><p> Заключение: выявленные особенности, связанные с влиянием на клиническую картину ПТСР коморбидных психических расстройств, следует анализировать в контексте взаимосвязей с тяжестью и интенсивностью влияния травматических событий на личностный профиль, а также учитывать при разработке лечебно-реабилитационных программ и динамической оценке эффективности терапии.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: PTSD in combatants is characterized by high comorbidity with mental disorders of the organic, affective, anxiety and personality spectrums. It is relevant to study the influence of comorbidity on the clinical picture and severity of PTSD manifestations.</p></sec><sec><title>The Aim of study</title><p>The Aim of study: to analyze the structure and severity of clinical and psychopathological characteristics of PTSD taking into account comorbidity and to establish relationships with the intensity of subjective experience of a traumatic event.</p></sec><sec><title>Patients and Methods</title><p>Patients and Methods: the study included male participants of the Special military operation (n = 52) aged 22 to 55 years (mean age 38.2 ± 8.9 years), who independently sought and underwent inpatient treatment for PTSD comorbid with other mental disorders (ICD-10 criteria). The diagnosis of PTSD was additionally verified by the international neuropsychiatric interview M.I.N.I. for PTSD. The severity of clinical manifestations of PTSD and the intensity of the impact of the traumatic event were assessed using the Mississippi PTSD Scale and the Traumatic Event Impact Rating Scale (TEIRS).</p></sec><sec><title>Results</title><p>Results: a predominance of other anxiety disorders, adjustment disorders (n = 24; 46.2%) and organic mental disorders (n = 12; 23.1%) was revealed in the structure of mental disorders comorbid with PTSD. The conducted factor analysis of PTSD symptoms made it possible to identify four groups of factors reflecting: (1) intrusive manifestations with pronounced anxiety, recurrence of experience and avoidance of the traumatic event; (2) negative emotional self-perception; (3) inability to experience positive emotions; (4) sleep disorders with hyperarousal. A statistically significant highest intensity of the impact of a traumatic event on the personality profile of combatants and the severity of PTSD manifestations was established in cases of comorbidity with anxiety, affective and personality disorders (p = 0.00001).
