Суицидальное поведение ассоциировано с активацией системного иммуновоспалительного ответа. Роль системного воспаления при НССП изучена в меньшей степени, а результаты проведенных исследований не систематизированы.</p><p>Цель обзора предметного поля: обобщить результаты исследований маркеров системного воспаления (МСВ) при НССП, ответив на вопросы: 1) существует ли связь МСВ с НССП? 2) зависит ли эта связь от клинических, социально-демографических, медико-биологических и фармакогенных факторов? 3) оценивалась ли прогностическая ценность МСВ при НССП?</p></sec><sec><title>Материал и методы</title><p>Материал и методы: в обзор предметного поля включали только оригинальные исследования МСВ у лиц с НССП, результаты которых были опубликованы до 31 марта 2025 г. Поиск релевантных источников проводили в базах данных MEDLINE и eLibrary.</p></sec><sec><title>Результаты</title><p>Результаты: из 427 публикаций отобрано 6 исследований (5 кросс-секционных и 1 лонгитюдное), отвечавших критериям соответствия. Анализ их результатов выявил: 1) повышенные уровни МСВ при НССП как у подростков, так и у взрослых пациентов (преимущественно с расстройствами аффективного спектра) по сравнению с пациентами без НССП и со здоровыми испытуемыми (4 исследования); 2) наличие единичных данных о влиянии на уровень МСВ таких факторов как жестокое обращение в детстве (1 исследование), суицидные попытки в анамнезе (1 исследование), тяжесть депрессии (1 исследование), паттерны ЭЭГ-активности (1 исследование); 3) связь повышенного уровня интерлейкина-6 у подростков с НССП с увеличением частоты самоповреждений в течение последующих 3 месяцев (1 исследование).</p></sec><sec><title>Заключение</title><p>Заключение: результаты обзора убедительно подтверждают связь НССП с изменениями профиля МСВ, отражающими активацию системного воспаления. Однако малое число исследований, их гетерогенность и преобладание кросс-секционного дизайна не позволяют однозначно оценить специфичность выявленных ассоциаций, причинно-следственные связи, влияние модифицирующих факторов и клиническую применимость МСВ как биомаркеров риска или прогноза НССП, что требует проведения исследований более высокого методологического качества.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: non-suicidal self-injury (NSSI) is a widespread phenomenon, particularly among adolescents, being a significant predictor of suicide. It is shown that suicidal behavior is associated with activation of the systemic immune-inflammatory response. The role of systemic inflammation in NSSI has been studied to a lesser extent, and findings from existing research have not been systematized.</p><p>Aim of the scoping review is to summarize findings from the studies that investigated systemic inflammatory markers (SIM) in NSSI, addressing the following questions: 1) Is there an association between SIM and NSSI? 2) Does this association depend on clinical, socio-demographic, biological, and treatment factors? 3) Has the prognostic value of SIM in NSSI been assessed?</p></sec><sec><title>Methods</title><p>Methods: only original studies investigating SIM in individuals with NSSI and published before March 31, 2025, were included. Searches for relevant publications were conducted in the MEDLINE and eLibrary databases.</p></sec><sec><title>Results</title><p>Results: in total, 429 publications were identified, 6 studies were included (5 cross-sectional and 1 longitudinal). Their results demonstrate: 1) higher levels of SIM in adolescents and adults (primarily with affective disorders) exhibiting NSSI compared to both patients without NSSI and healthy controls (4 studies); 2) sparse findings regarding the influence of the following factors on SIM levels: childhood maltreatment (1 study), history of suicide attempts (1 study), depression severity (1 study), and EEG activity patterns (1 study); 3) association between higher interleukin-6 levels in adolescents with NSSI with higher frequency of self-injurious episodes over the following 3 months (1 study).</p></sec><sec><title>Conclusions</title><p>Conclusions: the scoping review provides clear evidence for an association between NSSI and changes in SIM reflecting systemic inflammation activation. However, the small number of the studies, their heterogeneity, and mostly cross-sectional design preclude definitive conclusions regarding the specificity of the identified associations, causal relationships, the influence of modifying factors, and the clinical utility of SIM as biomarkers for NSSI risk or prognosis.
