В то же время мало данных о том, как когнитивно-поведенческая терапия (КПТ) влияет именно на социальную ангедонию и связанные с ней показатели качества жизни по SF-36, а также о том, как изменения социальной ангедонии соотносятся с динамикой депрессивной симптоматики и субъективного восприятия качества жизни. Цель исследования: оценить клинико-психологическую эффективность КПТ при депрессии через анализ динамики социальной ангедонии и качества жизни, а также изучить их взаимосвязь. Пациенты и методы: в проспективное исследование включены 56 пациентов с депрессией, находившиеся на амбулаторном или стационарном лечении. Основная группа — 34 пациента — проходила краткосрочную групповую КПТ в сочетании с фармакотерапией. В группе сравнения (контроля внимания) 22 пациентам в дополнение к фармакотерапии проводились регулярные поддерживающие встречи как терапевтическое вмешательство, однако без применения протокола КПТ. До и после лечения оценивали клинические и психологические показатели (MADRS, RSAS и SF-36). Анализ включал парные и межгрупповые сравнения и корреляции (p &lt; 0,05). Результаты: в группе КПТ отмечено значительное снижение суммарного балла MADRS с 30,4 до 14,8 (Δ = −15,6; p &lt; 0,001), клинический ответ — в 55,9%, ремиссия — в 29,4%. В группе сравнения снижение ограничивалось Δ = −2,4 (p = 0,011), без формирования терапевтиче- ского ответа и ремиссии. Показатель социальной ангедонии (Revised Social Anhedonia Scale, RSAS) в группе КПТ уменьшился с 19,44 до 14,53 балла (p &lt; 0,001), в группе сравнения снизился незначимо. Показатель качества жизни по SF-36 улучшился преимущественно в основной группе. Более выраженное уменьшение выраженности ангедонии в группе КПТ было связано с увеличением показателей ментального (MCS) компонента здоровья (ρ = −0,54), психического здоровья (MH) (ρ = −0,46) и физического (PCS) здоровья (ρ = −0,42). Выводы: КПТ приводит к ослаблению выраженности депрессии, уменьшению социальной ангедонии и улучшению качества жизни. Динамика ангедонии сильнее сопряжена с улучшением качества жизни, чем с изменением уровня депрессии, что подчеркивает ее значение как дополнительного клинического исхода.</p></abstract><trans-abstract xml:lang="en"><p>Background: depressive disorders are associated with reduced quality of life and impaired social functioning, with social anhedonia considered a key component of depression and as a factor of unfavorable course and prognosis. At the same time, there is limited evidence on how cognitive behavioral therapy (CBT) specifically affects social anhedonia and related quality of life indicators assessed by the SF-36, as well as on how changes in social anhedonia relate to the dynamics of depressive symptoms and subjective quality of life. The aim of study was to evaluate the clinical and psychological effectiveness of CBT in depression by analyzing changes in social anhedonia and quality of life, and to examine their interrelationships. Patients and Methods: this prospective study included 56 patients (34 in the CBT group and 22 in comparison group) receiving outpatient or inpatient care. The main group underwent short-term group CBT in combination with pharmacotherapy. The attention-control group received pharmacotherapy and regular supportive meetings perceived as a therapeutic intervention, but without the use of a CBT protocol. Clinical and psychological measures (MADRS, RSAS, and SF-36) were assessed before and after treatment. The analysis included within-group and between-group comparisons and correlation analyses (p &lt; 0.05). Results: in the CBT group, MADRS scores decreased significantly from 30.4 to 14.8 (Δ = −15.6; p &lt; 0.001); 55.9% of patients achieved a clinical response and 29.4% achieved remission. In the control group, the reduction was limited (Δ = −2.4; p = 0.011), with no responses or remissions. Social anhedonia (RSAS) decreased in the CBT group from 19.44 to 14.53 (p &lt; 0.001) and showed no significant change in the control group. Quality of life assessed by the SF-36 improved predominantly in the CBT group. A greater reduction in social anhedonia in the CBT group was associated with increases in the mental component summary (MCS) (ρ = −0.54), mental health (MH) (ρ = −0.46), and physical component summary (PCS) (ρ = −0.42). Conclusions: CBT is associated with a reduction in depressive symptoms, a decrease in social anhedonia, and improvements in quality of life.
