В научной литературе обсуждается возможность использования в качестве маркеров воспаления соотношений нейтрофилов, лимфоцитов и тромбоцитов, информация о содержании которых может быть получена рутинными методами общего анализа крови. Цель исследования: изучить у больных алкогольной зависимостью уровень нейтрофильно-лимфоцитарного отношения (НЛО), тромбоцитарно-лимфоцитарного отношения (ТЛО), индекса системного иммунного воспаления (ИСВ), полученных на основе общего анализа крови, и оценить их связь с длительностью заболевания и соматической отягощенностью. Пациенты, группа контроля: обследованы 95 мужчин в возрасте 30–60 лет с диагнозом «Психические и поведенческие расстройства в результате употребления алкоголя» (синдром зависимости — F10.21 и синдром отмены — F10.30 по МКБ-10) и 26 условно здоровых мужчин. Методы: клинический анализ крови проводили на гематологическом анализаторе Mindray BC-3600. Образцы крови пациентов брали после проведения алкогольной детоксикации. По абсолютным значениям индивидуальных гемограмм вычисляли НЛО, ТЛО, ИСВ. Результаты: у пациентов общей группы по отношению к контрольной группе здоровых снижено количество тромбоцитов, повышено значение НЛО. Наибольшее повышение НЛО установлено в группе пациентов с давностью заболевания более 10 лет, сочетанной соматической патологией 3–4 систем организма. Наибольшая соматическая отягощенность обнаружена у пациентов со значением ИСВ 500 и выше. Заключение: наиболее значимым маркером воспаления при алкогольной зависимости является НЛО. ИСВ выше 500 возможно использовать для стратификации пациентов с выраженной соматической отягощенностью, назначения углубленного обследования и адекватного лечения на ранних этапах госпитализации.</p></abstract><trans-abstract xml:lang="en"><p>Background: the complexity of the pathogenesis of alcohol dependence is due to the specific effect of ethanol on brain structures with the formation of dependence and toxic effects on systems and organs, accompanied by inflammation and somatic pathology. The possibility of using the ratios of neutrophils, lymphocytes and platelets as markers of inflammation, information on the content of which can be obtained by routine methods of a complete blood count, are discussed in literature. The aim was to study the level of neutrophil-lymphocyte ratio (NLR), platelet-lymphocyte ratio (PLR), systemic immune inflammation index (SII) in patients with alcohol dependence, obtained on the basis of a complete blood count, and to assess their relationship with the duration of the disease and somatic burden. Patients, control group: 95 men aged 30–60 years with the ICD-10 diagnosis of Mental and behavioral disorders due to alcohol use (dependence syndrome — F10.21 and withdrawal syndrome — F10.30) and 26 conditionally healthy men were examined. Methods: clinical blood analysis was performed on BC-3600 hematology analyzer. Blood was taken from patients after alcohol detoxification. Based on the absolute values of individual's hemogram, the NLR, PLR, and SII were calculated. Results: in patients with alcohol dependence, in relation to the control group, the platelet count was reduced, the NLR value was increased. The greatest increase in NLR was found in the group of patients with disease duration of more than 10 years, with combined somatic pathology of 3–4 body systems. Patients with SII 500 and above were characterized by the greatest somatic burden.
