Материал и метод: по ключевым словам «депрессии», «поздний возраст» «патогенез» проводился поиск научных статей в базах данных MEDLINE/PubMed за 2000–2020 гг. Заключение: несмотря на большое число гипотез патогенеза депрессий, они являются не взаимоисключающими, а скорее взаимодополняющими, что отражает системную многофакторную природу депрессивных расстройств, в первую очередь депрессий позднего возраста. </p><p>Многие из этих гипотез тесно связаны между собой. Правомерно предположить, что депрессия в позднем возрасте — это гетерогенный клинический синдром с множественной этиологией депрессивных расстройств и вовлечения различных патогенетических механизмов. Изучение особенностей патогенеза депрессивных расстройств в позднем возрасте позволяет выявить важные и перспективные области для дальнейших исследований с целью разработки персонифицированных терапевтических подходов и выделения предикторов терапевтического ответа. Подбор адекватной психофармакотерапии в этой возрастной группе особенно актуален по отношению к часто рецидивирующим и резистентным к терапии депрессиям, что является факторами неблагоприятного прогноза заболевания в целом.</p></abstract><trans-abstract xml:lang="en"><p>Objective: to present a review of domestic and foreign modern studies highlighting the features of the pathogenesis of depressive disorders in late age. Material and method: using the keywords «depression», «late age», «pathogenesis», scientific articles were searched in the MEDLINE/PubMed databases for the period 2000–2020. Conclusion: Despite the large number of hypotheses regarding the pathogenesis of depression, they are not mutually exclusive, but rather complementary, which reflects the systemic multifactorial nature of depressive disorders, primarily late-age depression. However, many of these hypotheses are closely related. It is likely to suggest that late-life depression is a heterogeneous clinical syndrome with multiple etiologies of depressive disorders, and the involvement of various pathogenetic mechanisms. The study of the pathogenesis of depressive disorders in late life reveals important and promising areas for further research in order to develop personalized therapeutic approaches and identify predictors of therapeutic response. Selection of adequate psychopharmacotherapy in this age group is especially relevant in relation to often recurrent and therapy-resistant depressions, which are factors of unfavorable prognosis of the disease as a whole.</p></trans-abstract><kwd-group xml:lang="ru"><kwd>депрессивные расстройства</kwd><kwd>поздний возраст</kwd><kwd>патогенез</kwd></kwd-group><kwd-group xml:lang="en"><kwd>depressive disorders</kwd><kwd>old age</kwd><kwd>pathogenesis</kwd></kwd-group></article-meta></front><back><ref-list><title>References</title><ref id="cit1"><label>1</label><citation-alternatives><mixed-citation xml:lang="ru">Мосолов СН.
