Общая заболеваемость БА резко возрастает, через два десятилетия удвоится и достигнет 100 млн случаев во всем мире. Поэтому разработка модифицирующей болезнь терапии, способной задержать или даже предотвратить начало и прогрессирование БА, стала мировым приоритетом.</p></sec><sec><title>Цель</title><p>Цель: представить обзор отечественных и зарубежных современных исследований, освещающих вопросы патогенеза БА и модифицирующей болезнь терапии.</p></sec><sec><title>Материал и методы</title><p>Материал и методы: по ключевым словам «болезнь Альцгеймера, поздний возраст, легкие когнитивные нарушения, депрессия, терапия, церебролизин, эффективность» проводился поиск научных статей в базах данных MEDLINE, PUBMED за период 1980–2020 гг.</p></sec><sec><title>Результаты и выводы</title><p>Результаты и выводы: поскольку патофизиология БА многофакторна, неудивительно, что все попытки изменить течение заболевания с помощью препаратов, направленных на единую терапевтическую цель, оказались безуспешными. Таким образом, комбинированная мультимодальная терапия с использованием нескольких препаратов с единым механизмом действия или многоцелевых препаратов представляется наиболее перспективной стратегией как для эффективной терапии БА, так и для ее профилактики. Церебролизин, действуя как мультимодальный пептидергический препарат с доказанным нейротрофическим действием, оказывает не только немедленное терапевтическое воздействие на БА, что может отражать его потенциальную пользу для модификации течения заболевания. Многочисленные клинические испытания показали, что церебролизин безопасен и эффективен при лечении БА, а также может усиливать и продлевать эффективность холинергических препаратов, особенно у пациентов с БА средней степени тяжести. В настоящем обзоре мы обобщаем достижения в изучении терапевтической значимости препарата и его влияния на патогенез течения БА, уделяя особое внимание механизмам нейротрофического действия. В обзоре представлены результаты как доклинических, так и клинических исследований церебролизина при лечении БА и додементных когнитивных расстройств, а также поздних депрессий.</p></sec></abstract><trans-abstract xml:lang="en"><sec><title>Background</title><p>Background: Alzheimer’s disease (AD) is the most common neurocognitive disorder and a global health problem. The prevalence of AD is increasing dramatically, and will double in two decades to reach 100 million cases worldwide. Therefore, the development of disease-modifying therapies that can delay or even prevent the onset and progression of AD has become a global priority.</p></sec><sec><title>Objective</title><p>Objective: to present a review of domestic and foreign modern studies covering the pathogenesis of AD and disease-modifying therapy.</p></sec><sec><title>Material and methods</title><p>Material and methods: the keywords “Alzheimer’s disease, late age, mild cognitive impairment, depression, therapy, cerebrolysin, effectiveness” were used to search for scientifi c articles in MEDLINE and PUBMED databases for the period 1980–2020.</p></sec><sec><title>Results and conclusions</title><p>Results and conclusions: since the pathophysiology of AD is multifactorial, it is not surprising that all attempts to change the course of the disease with drugs aimed at a single therapeutic goal were unsuccessful. Thus, combined multimodal therapy using several drugs with a single mechanism of action or multi-purpose drugs seems to be the most promising strategy for both effective therapy of AD and its prevention. Cerebrolysin, acting as a multimodal peptidergic drug with a proven neurotrophic effect, has not only an immediate therapeutic effect on AD, which may refl ect its potential benefi t for modifying the course of the disease. Numerous clinical trials have shown that cerebrolysin is safe and effective in the treatment of AD, and can also enhance and prolong the effectiveness of cholinergic drugs, especially in patients with moderate AD. In this review, we summarize the achievements in the study of the therapeutic signifi cance of the drug and its effect on the pathogenesis of AD, paying special attention to the mechanisms of neurotrophic action.
