Поведенческие и психопатологические симптомы при болезни Альцгеймера: эпидемиологические, психопатологические и нейробиологические аспекты

Аннотация
Цель обзора. Представить современное состояние проблемы поведенческих и психических симптомов деменции (ППСД) по данным научных публикаций.Материал и метод. Проведен анализ опубликованных исследований, посвященных изучению ППСД при болезни Альцгеймера и других видах деменций позднего возраста. Результаты. Исследователями проблемы деменций единодушно признается, что ППСД являются важной составной частью клинической картины болезни Альцгеймера. Они включают такие некогнитивные симптомы, как агрессия, двигательное беспокойство, блуждание, бесцельная активность, расторможенность, апатия, аффективные нарушения, бред, галлюцинации, а также расстройства сна и аппетита. Показано, что один или более из этих симптомов наблюдается у 61-92% пациентов с деменцией, а их частота возрастает по мере прогрессирования деменции. Признается, что ППСД являются ведущей причиной стресса и повышенной нагрузки для лиц, ухаживающих за больными, а также увеличивают риск госпитализации пациента с деменцией. В обзоре рассмотрены предложенные авторами патогенетические концепции ППСД, в настоящее время далекие от окончательной разработки. Наиболее вероятным представляется, что ППСД возникают в результате сложного взаимодействия психологических, социальных и биологических факторов.Выводы. Быстрая и комплексная оценка ППСД необходима в постановке диагноза деменции и планировании соответствующего лечения, направленного на облегчение страдания пациентов и членов их семей.
